Understand the client first

"Before we get started, I want to understand what brought you in."

  • What are their primary fitness goals?
  • Why are those goals important to them?
  • What have they tried before?
  • What challenges have kept them from reaching those goals?
  • What would success look like in the next 3–6 months?

Establish a baseline

  • Body measurements/body composition when appropriate
  • Movement and fitness assessment
  • Strength, mobility, cardiovascular/endurance baseline
  • Training history, lifestyle and current routine

"This gives us a starting point so we can actually measure your progress."

Set expectations

  • Their program will be individualized—not a generic workout.
  • Sessions will have a purpose tied to their goals.
  • You'll progressively challenge them as they improve.
  • You'll educate them on what they're doing and why.
  • Progress will be reassessed over time.

"I'll provide the structure and accountability, but your consistency inside and outside our sessions is what will drive the results."

Transition into the training package

  • Connect the recommendation directly to what they told you.

"Based on your goal of losing body fat while getting stronger, I'd recommend training together ___ times per week. That gives us enough consistency to work toward the goal while still giving you recovery and independent training days."

Then present the appropriate package rather than listing every option immediately.

Close with a question

  • "How does that approach sound to you?"
  • Then: "Are you ready to get started?"
  • Or softer: "Would you like me to walk you through the training options that would fit that plan?"

I, the undersigned, have read, understood to my full satisfaction, and completed this questionnaire. I acknowledge this clearance is valid for a maximum of 12 months and becomes invalid if my condition changes.
